- Applications received during the first quarter of the year, from January 1 through March 31, will pay the full fee and receive benefits through the end of the current year, December 31
- Applications received from April 1 through September 31 will pay the prorated fees above and receive benefits through the end of the current year, December 31
- Applications received during the last quarter of the year, October 1 through December 31, will pay the full fee and receive benefits through December 31 of the following year
